1. Responsibility for the animal

I understand that the behaviour of any dog or other animal cannot be guaranteed and may sometimes be unpredictable. I remain responsible for the supervision, handling, control, care, health, welfare and behaviour of the animal named in this application.

I agree to take reasonable precautions and accept responsibility for any injury, loss, damage, cost or liability caused by the animal where I am legally responsible. The animal must remain appropriately controlled, must not present an unreasonable risk, must behave appropriately in public and must be cared for in accordance with applicable law.

To the fullest extent permitted by law, Assistance Dogs UK, Assistance Support Animal Registry UK and their employees, officers, directors, agents and contractors will not be responsible for injury, loss, damage, claims, costs or expenses arising from the behaviour, handling, ownership, training or presence of my animal, except where liability cannot lawfully be excluded or where loss results from the organisation’s own negligence or legal breach.

2. Training and registration status

I confirm that I have accurately stated whether the animal was trained by an organisation, an independent trainer, an owner or handler, is currently undergoing training, or has not received task-specific or public-access training.

I understand that registration does not turn an untrained animal or ordinary pet into a trained assistance dog. Registration does not certify an animal as professionally trained, task-trained or suitable for unrestricted public access unless that description is accurate and supported.

3. Purpose and limitations of registration

I understand that registration is a voluntary record of the information supplied. It is not a compulsory government registration or licence, does not independently prove disability or training, does not guarantee access and does not replace assessments or evidence lawfully required by businesses, landlords, employers, education providers or transport operators.

Identification cards, certificates, letters, harnesses, leads, badges, lanyards and vests do not independently create legal status or guarantee access. Rights and reasonable adjustments depend on the individual circumstances and applicable law.

4. Accuracy and evidence

I confirm that all information and evidence supplied is complete, current and accurate to the best of my knowledge. Assistance Dogs UK may reasonably verify submitted evidence. False, misleading, incomplete, altered or fraudulent information may result in refusal, suspension or cancellation.

5. Personalised goods, services and cancellation

I understand that identification cards, certificates, letters, portraits and other items containing individually selected information are personalised or made to my specifications. I expressly request that review, registration, digital-document and personalised-item work begins after my application and payment are accepted.

Correctly produced personalised goods cannot ordinarily be cancelled, returned or refunded merely because I no longer want them. This does not affect statutory rights where an item is faulty, damaged, incorrectly produced, not as described or otherwise fails to meet legal requirements.

I must check all information before submission and payment. Assistance Dogs UK is not responsible for errors in personalised items where those items accurately reproduce the information I supplied, although corrected replacements may be available at additional cost.

6. Applicant’s confirmation

By submitting the application, I confirm that I have read and understood the terms, accurately declared training status, understand the difference between registration and training, accept responsibility for the animal and expressly request that registration services and personalised and digital production begin after acceptance and payment.
